A company is looking for a Case Manager RN (Remote, Nevada RN
licensure required).
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
for members
Develop proactive care plans to enhance member wellness and
outcomes
Utilize clinical tools and data to evaluate member needs and
facilitate referrals
Required Qualifications and Education
Active and unrestricted Nevada RN licensure
3+ years of clinical experience
2+ years of Case Management, Discharge Planning, or Home Healthcare
Coordination experience
Bachelor's degree or associate degree with equivalent
experience
