Facility in Grand Rapids, WI seeking Ophthalmology locums coverage Details:

Practice Setting: Hospital-based
Reason for Coverage: Trauma
Dates of Coverage: January - July
Call Schedule: One week a month
Average Number of Patients (Hospital): 1-2 per month
Patient Population: 100% Adult/Geriatric
Practice Breakdown:
- 50% Non-Surgical
- 50% Surgical/Laser
- EMR System: Epic
- Trauma Level: Level 2
- Required Clinical Areas: Non-Surgical and Surgical/Invasive Credentialing Information:
  - Estimated Timeframe: 3 months Requirements:
    - Board Certified Ophthalmologist Required
    - MI License Required
    - BLS/ACLS Not required unless performing moderate or deep sedation Facility Information:
      - Unit/Floor: Ophthalmology
